Cacodylic acid is used as a non-selective herbicide, defoliant, and silvicide (Hartley & Kidd, 1987; Morgan, 1989).

GENERAL CLINICAL EFFECTS

Highly toxic, may be fatal if inhaled, swallowed or absorbed through skin. Avoid any skin contact. Effects of contact or inhalation may be delayed. Fire may produce irritating, corrosive and/or toxic gases. Runoff from fire control or dilution water may be corrosive and/or toxic and cause pollution.
ACUTE CLINICAL EFFECTS
- Forestry workers using arsenic compounds, primarily cacodylic acid and monosodium methane-arsonate for thinning forests, reported anorexia, nausea, and abdominal pain (Finkel, 1983).
CHRONIC CLINICAL EFFECTS
- In chronically exposed experimental animals, acute renal tubular necrosis has been noted. Weight loss or decreased weight gain and death have also occurred (RTECS , 1996; HSDB , 1996).

SUMMARY
- FIRST AID - EMERGENCY RESPONSE GUIDEBOOK, GUIDE 151 (ERG, 2004)
Move victim to fresh air. Call 911 or emergency medical service. Give artificial respiration if victim is not breathing. Do not use mouth-to-mouth method if victim ingested or inhaled the substance;give artificial respiration with the aid of a pocket mask equipped with a one-way valve or other proper respiratory medical device. Administer oxygen if breathing is difficult. Remove and isolate contaminated clothing and shoes. In case of contact with substance, immediately flush skin or eyes with running water for at least 20 minutes. For minor skin contact, avoid spreading material on unaffected skin. Keep victim warm and quiet. Effects of exposure (inhalation, ingestion or skin contact) to substance may be delayed. Ensure that medical personnel are aware of the material(s) involved and take precautions to protect themselves.
INHALATION EXPOSURE - DERMAL EXPOSURE - EYE EXPOSURE - DECONTAMINATION: Remove contact lenses and irrigate exposed eyes with copious amounts of room temperature 0.9% saline or water for at least 15 minutes. If irritation, pain, swelling, lacrimation, or photophobia persist after 15 minutes of irrigation, the patient should be seen in a healthcare facility.
ORAL EXPOSURE - If swallowed and victim is conscious, have victim drink water or milk; induce vomiting (CHRIS , 1992). If swallowed and victim is unconscious or having convulsions, keep victim warm and wait for emergency medical assistance (CHRIS , 1992).
ARSENIC POISONING - The following treatment information is for Arsenic. ORAL EXPOSURE - GASTRIC DECONTAMINATION - Aggressive decontamination with gastric lavage is recommended. Do not induce emesis. If X-ray demonstrates arsenic in the lower gastrointestinal tract, whole bowel irrigation should be considered. Activated charcoal may not bind significant amounts, but is recommended until definitive quantitated data are available. Fluid repletion should begin as soon as possible. GASTRIC LAVAGE: Consider after ingestion of a potentially life-threatening amount of poison if it can be performed soon after ingestion (generally within 1 hour). Protect airway by placement in the head down left lateral decubitus position or by endotracheal intubation. Control any seizures first. ACTIVATED CHARCOAL: Administer charcoal as a slurry (240 mL water/30 g charcoal). Usual dose: 25 to 100 g in adults/adolescents, 25 to 50 g in children (1 to 12 years), and 1 g/kg in infants less than 1 year old. HYPOTENSION: Infuse 10 to 20 mL/kg isotonic fluid. If hypotension persists, administer dopamine (5 to 20 mcg/kg/min) or norepinephrine (ADULT: begin infusion at 0.5 to 1 mcg/min; CHILD: begin infusion at 0.1 mcg/kg/min); titrate to desired response. Monitor liver, renal, and cardiac functions. Maintain high urine output. URINE ALKALINIZATION - may prevent the deposition of red cell breakdown products from hemolysis in the renal tubules. URINE ALKALINIZATION - Assure adequate hydration and renal function. Administer 88 to 132 mEq/L sodium bicarbonate and 20 to 40 mEq KCL (as needed) in D5W or other fluid to produce a urine pH of at least 7.5. Monitor fluid balance and serum potassium. Obtain hourly intake/output and urine pH. CHELATION THERAPY - BAL - Symptomatic patients should be treated with BAL 3 to 5 mg/kg/dose IM every 4 to 12 hours. The dose and frequency depend on the degree of toxicity seen. Higher doses of BAL invariably cause adverse effects. PENICILLAMINE - As symptoms and signs subside, change to oral D-penicillamine 100 mg/kg/day up to 2 g daily in four divided doses. If allergic to penicillamine, administer BAL for 5 days with tapering of the dose. In severely ill patients, combined therapy with both BAL and D-penicillamine should be considered. ENDPOINT - Chelation therapy should be stopped when the urinary arsenic level falls below 50 mcg per 24 hours. If renal failure exists, dose of either chelator should be adjusted downward after loading dose. DMSA - Dimercaptosuccinic acid (DMSA) is currently being investigated in the United States. It may be more effective and cause fewer side effects than BAL.
FLUID BALANCE - Monitor volume status; establish adequate urine flow of at least 1 to 2 mL/kg/hr. X-RAY - Arsenic is radiopaque. Obtain abdominal film and repeat as necessary to insure that gastric emptying maneuvers have been effective. A chest radiograph should also be obtained, as pulmonary edema may occur.
When the arsenic exposure has been environmental, removing the patient from the source of exposure is beneficial. Children with elevated urinary arsenic concentrations above normal had a substantial reduction in arsenic concentrations after moving away from an area where the soil had been contaminated by a smelter (MMWR, 1987).
-RANGE OF TOXICITY
MINIMUM LETHAL EXPOSURE
The minimum lethal human dose to this agent has not been delineated. The following information is for ARSENIC: One milligram/kilogram of ingested arsenic may be lethal in a child (Woody & Kometani, 1948). As little as 20 milligrams of arsenic may produce life-threatening toxicity (Zaloga et al, 1970; Schoolmeester & White, 1980; Hutton & Christians, 1983).
Arsenic trioxide in a solubilized form becomes sodium arsenite, which is more toxic than in an unsolubilized form.

It may provide little or no thermal protection. REACTIVITY HAZARD
- Cacodylic acid is hygroscopic (Budavari, 1989).
- Cacodylic acid is hazardous when water solution is in contact with active metals (ie, iron, aluminum, and zinc) (Sax & Lewis, 1989).
