USES/FORMS/SOURCES
It is used as a rodenticide and in anticoagulation medical therapy (Budavari, 1989; ACGIH, 1992; EPA, 1985; Baselt & Cravey, 1989). Warfarin sodium is the most frequently used coumarin derivative for oral anticoagulation in humans (HSDB, 1996).
Warfarin sodium, also called COUMADIN, is a synthetic vitamin K antagonist. It occurs as a colorless crystalline powder solid, and may be tasteless or have a slightly bitter taste (Budavari, 1996; ACGIH, 1992; EPA, 1985). It is soluble in water and alcohol, and slightly soluble in chloroform and ether (Budavari, 1996). Because both warfarin and its sodium salt are called COUMADIN, there is some confusion in the literature about which substance is being discussed. Warfrin and its sodium salt should be in the same form once absorbed systemically.
-CLINICAL EFFECTS
GENERAL CLINICAL EFFECTS
- Warfarin sodium is a synthetic vitamin K antagonist used as a rodenticide and in anticoagulation therapy. It may be absorbed following ingestion, inhalation, or dermal contact.
- The primary effect of warfarin overexposure is prolongation of prothrombin time, with subsequent risk of hemorrhage. Increased prothrombin time from a toxic dose usually appears within 24 hours of ingestion and peaks between 36 to 72 hours.
- Toxicity to the fetus has been reported; it is recommended that warfarin not be used in either early pregnancy or after the 32nd week of pregnancy.

Highly toxic, may be fatal if inhaled, swallowed or absorbed through skin. Avoid any skin contact. Effects of contact or inhalation may be delayed. Fire may produce irritating, corrosive and/or toxic gases. Runoff from fire control or dilution water may be corrosive and/or toxic and cause pollution.
ACUTE CLINICAL EFFECTS
- Based on its acute oral LD50 of 8.7 mg/kg in rats (RTECS , 1996), warfarin sodium is a HIGHLY TOXIC substance. It can be absorbed in toxic amounts by the dermal route (Fristedt & Sterner, 1965). Its major effect is prolongation of prothrombin time, leading to hemorrhage (Budavari, 1996; Baselt & Cravey, 1989). It may also cause tissue necrosis (PDR, 1993).
- The kinetics of appearance of prolonged prothrombin time correspond to the half-life of factor VII, whose production is blocked by warfarin sodium (Morgan, 1989; Kumer & Nwangwu, 1981). Increase in prothrombin time usually appears within 24 hours of overexposure, and peaks between 36 to 72 hours (Kumer & Nwangwu, 1981).
- Indirect effects of warfarin sodium overexposure are consequences of uncontrolled bleeding, and include hypotension (low blood pressure), hemorrhage in the retina of the eye, upper airway occlusion and asphyxiation due to hemorrhage into the soft tissues of the neck, hemoptysis (coughing up blood), and paralysis from intracerebral hemorrhage (Nagle, 1970; Grant, 1986; Lepore, 1976; Boster & Bergin, 1983; Kumer & Nwangwu, 1981; EPA, 1985).
- Other results of bleeding are bloody or dark stools, easy bruising, abdominal and back pain, shock, and rupture of the spleen (Kumer & Nwangwu, 1981; Nagle, 1970; Soyer et al, 1976).
CHRONIC CLINICAL EFFECTS
- Warfarin sodium has CUMULATIVE TOXICITY, due to progressive diminution of factors VII, IX, X, and anticoagulant proteins, resulting from antagonism of vitamin K by warfarin (Majerus et al, 1990; PDR, 1993). Vitamin K is required for synthesis of the coagulation factors and anticoagulant proteins (Majerus et al, 1990). Chronic exposure to warfarin sodium causes the same type of coagulation defect as acute overexposures, but the magnitude and severity may be greater. Virtually all studies of chronic exposure to warfarin sodium are of its therapeutic use.
- Deaths have occurred from eating cornmeal containing 0.25% sodium warfarin (Finkel, 1983; Morgan, 1989; Baselt & Cravey, 1989; Lisella et al, 1971). A total dose of 1 gram, ingested over 13 days, was fatal on the 15th day (Baselt & Cravey, 1989).
- Various bleeding complications have been reported from chronic therapeutic administration of warfarin sodium, including epistaxis, hemoptysis, hematuria, gingival bleeding, hematochezia, guiac positive stool, bruising, vaginal bleeding, subconjunctival hemorrhage, and hematospermia (Gurwitz et al, 1988).
- Warfarin sodium has caused abnormal bleeding following chronic application to the skin in contaminated talc (Martin-Bouyer et al, 1983; Dreyfus et al, 1983).
- Chronic exposure to warfarin sodium has produced alopecia and calcification of the cartilaginous rings in the trachea and bronchi in elderly patients (Umlas & Harken, 1988; Moncada et al, 1992). Dermal necrosis is an uncommon, but potentially fatal, side-effect of warfarin sodium therapy (Lacy & Goodin, 1975; Martin et al, 1970). In one review, skin necrosis was reported to occur at a frequency of 0.1% (Viegas, 1991).

Move victim to fresh air. Call 911 or emergency medical service. Give artificial respiration if victim is not breathing. Do not use mouth-to-mouth method if victim ingested or inhaled the substance;give artificial respiration with the aid of a pocket mask equipped with a one-way valve or other proper respiratory medical device. Administer oxygen if breathing is difficult. Remove and isolate contaminated clothing and shoes. In case of contact with substance, immediately flush skin or eyes with running water for at least 20 minutes. For minor skin contact, avoid spreading material on unaffected skin. Keep victim warm and quiet. Effects of exposure (inhalation, ingestion or skin contact) to substance may be delayed. Ensure that medical personnel are aware of the material(s) involved and take precautions to protect themselves.
INHALATION EXPOSURE - INHALATION: Move patient to fresh air. Monitor for respiratory distress. If cough or difficulty breathing develops, evaluate for respiratory tract irritation, bronchitis, or pneumonitis. Administer oxygen and assist ventilation as required. Treat bronchospasm with an inhaled beta2-adrenergic agonist. Consider systemic corticosteroids in patients with significant bronchospasm.
DERMAL EXPOSURE - EYE EXPOSURE - DECONTAMINATION: Remove contact lenses and irrigate exposed eyes with copious amounts of room temperature 0.9% saline or water for at least 15 minutes. If irritation, pain, swelling, lacrimation, or photophobia persist after 15 minutes of irrigation, the patient should be seen in a healthcare facility.

MINIMUM LETHAL EXPOSURE
- Death from severe hemorrhagic complications has been reported in persons who ate food made with warfarin sodium baited cornmeal. Ingestion of a total of 1 gram of warfarin over a 13-day period resulted in death.

MAXIMUM TOLERATED EXPOSURE
Usual PO maintenance range is 2 to 10 mg/day. A maintenance dose of 5 to 7.5 mg/day in adults is usually adequate to keep the prothrombin time at 2 to 3 times control. A maintenance dose of 5 to 7.5 mg/day in adults is usually adequate to keep the prothrombin time at 2 to 3 times control (ACGIH, 1986). Usual recommended initial or loading dose is 20 to 60 milligrams followed by maintenance dose, although loading doses are now no longer recommended. An estimated dose of 0.075 to 0.48 milligram/kilogram, in an adult may produce a peak total warfarin concentration of 0.5 to 3 micrograms/milliliter. This is associated with a prothrombin ratio value of 1.5 to 2.5.
INFANT Dermal exposure to talcum powder contaminated with 1.7 to 6.5% warfarin resulted in 177 infant deaths in Vietnam in 1981. Experimentally, a baboon exposed daily to talc containing 3% warfarin applied on the buttocks under a diaper, died a hemorrhagic death five days later (Martin-Bouyer et al, 1983).
ADULT A farmer who had dermal contact with warfarin sodium on multiple occasions developed bleeding lips, epistaxis, hematomas, and hematuria (Baselt & Cravey, 1989).

Wear positive pressure self-contained breathing apparatus (SCBA). Wear chemical protective clothing that is specifically recommended by the manufacturer. It may provide little or no thermal protection. Structural firefighters' protective clothing provides limited protection in fire situations ONLY; it is not effective in spill situations where direct contact with the substance is possible.
