Radium is used in physical research, in industrial radiography, and as a radiation source in antineoplastic treatment (Lewis, 1993; Budavari, 1996).
RADIUM-226 is a silver-white radioactive, luminescent alkaline-earth metal which blackens in air (Budavari, 1996; Lewis, 1993). It is a member of the uranium series of radioactive decay (Budavari, 1996; Lewis, 1993). Radium can occur as the chloride, bromide, carbonate, and sulfate salts (HSDB; (Budavari, 1996). Of its radioactive emissions, 94.3% are alpha-particles with an energy of 4.78 MeV, and the remainder are alpha-particles of 4.59 MeV and beta-quanta of 0.186 MeV (HSDB).
It is formed from URANIUM-238, which occurs naturally in low levels in PHOSPHATE ROCK and other geological materials. It forms RADON upon spontaneous disintegration, with a half-life of 1620 years (Budavari, 1996; Lewis, 1993). Since alpha-particles have rather poor penetrating ability, the major radiological hazard from exposure to Ra-226 is from INTAKE INTO THE BODY. Ra-226 is widespread in nature and occurs in drinking water (Hickey & Campbell, 1968), sometimes at high enough levels to increase the risk of bone cancer (Petersen, 1966). The average dose from radium in drinking water is about 6.4 mrem/year (0.0064 rem/year) (HSDB). Ra-226 can accumulate in barium and calcium sulfate scale from wet-process phosphoric acid plants (Lardinoye, 1982).

ACUTE CLINICAL EFFECTS
- Because of its intrinsic radioactivity, it is impossible to separate the toxic effects of radium-226 from those of IONIZING RADIATION. Radium is bone-seeking when systemically absorbed (Lewis, 1993). Acute exposure to high levels of ionizing radiation produces RADIATION SICKNESS, also known as ACUTE RADIATION SYNDROME. This condition occurs generally with acute exposures of greater than 100 RADs and has four distinct phases: prodrome, latent, manifest illness, and recovery.
- Other effects of acute exposure to ionizing radiation are dermal erythema (redness and swelling of tissue) and burns.
CHRONIC CLINICAL EFFECTS
- Radium itself deteriorates to form RADON-222 gas and RADON DAUGHTERS. There is a large human experience with radium in nuclear medicine and occupational exposure. Luminous dial painters, who were primarily women, received chronic oral doses of radium from licking paintbrushes to keep a fine point, resulting in cancer (see below under carcinogenic effects).
- Other diseases associated with chronic radium exposure include ANEMIA and APLASTIC ANEMIA (because of proximity of the radiation in the bone to the bone marrow). The experience with the radium-dial painters has proven that RADIUM TAKEN INTO THE BODY ORALLY CAN BE EFFECTIVELY ABSORBED AND SEEKS THE BONE as the primary target organ. Radium mimics CALCIUM and can replace calcium in the bone (Sax, 1984).
