Paraquat methosulfate may be used as a contact herbicide and desiccant (EPA, 1985) Sax & Lewis, 1989); however, this particular paraquat salt no longer appears to be sold in commercial products.
Paraquat methosulfate is a salt of PARAQUAT. It occurs as a colorless to white or yellowish, odorless, crystalline solid material which is incompatible with strong oxidizing agents (EPA, 1985) NIOSH, 1989). Liquid paraquat preparations may be cola-colored; a bluish dye, an emetic, and a stenching agent are added to all liquid paraquat formulations produced by ICI (Onyon & Volans, 1987; (Pond, 1990). Little information was found on the toxicity of paraquat methosulfate, apart from that of paraquat itself. This review is based on the properties of paraquat. Effects attributed specifically to paraquat methosulfate are identified.

ACUTE CLINICAL EFFECTS
- Based on its acute oral LD50 of 100 mg/kg in rats (RTECS , 1993), paraquat methosulfate is a moderately toxic substance. It may be absorbed following ingestion, inhalation, or dermal contact (EPA, 1985) Sax & Lewis, 1989). PARAQUAT is highly toxic to humans, however; ingestion of as little as 6 grams of paraquat ion may be fatal (EPA, 1985). Ingestion of 5 to 10 milliliters of the concentrate (the usual concentration is 200 g paraquat ion per liter) has caused death (Tsatsakis et al, 1996). In one small case series, all patients ingesting more than 100 milliliters of paraquat concentrate died (Tsatsakis et al, 1996). The reliability of such estimates is dependent on the accuracy of information often obtained under difficult circumstances.
- With inhalation exposure, epistaxis, headaches, cough, and sore throat may be noted (EPA, 1985; Garnier, 1995). No fatalities following inhalation exposure have been reported. Following ingestion of the concentrate, a burning sensation in the mouth, throat, and abdomen may occur, as well as vomiting, hematemesis, bloody diarrhea, and headache (EPA, 1985; Bismuth, 1995).
- The main target organ of paraquat toxicity is the LUNG, regardless of the route of exposure. Once a threshold dose has been absorbed, an irreversible disease process is initiated that involves a severe inflammatory reaction and fibrosis in the lung (Anon, 1968).
- The first signs and symptoms of paraquat poisoning are abdominal cramping, nausea, and vomiting. There may be diarrhea, fever, and hypotension. A dose as small as one sip has led to fatal failure of heart, liver, lungs, and kidneys (Lanzinger, 1969).
- Following ingestion of less than 20 to 30 mg of paraquat ion per kg, patients are asymptomatic or develop only vomiting and diarrhea. Full recovery is usual, but transient changes in lung function parameters may occur (Vale et al, 1987).
- The response to doses between 20 and 50 mg/kg of paraquat ion per kg appears to go through two phases. The first phase includes sore throat and difficulty swallowing during the first 24 hours, followed by acute kidney failure in 2 to 6 days, possible liver insufficiency, and early pulmonary involvement with dyspnea, rales, and pulmonary edema (Bismuth, 1995). Some patients die during this acute phase.
- The second phase involves the lungs and may last for 1 to 4 weeks after paraquat ingestion. There is rapid development of pulmonary fibrosis in this phase, which is usually fatal (Clayton & Clayton, 1982; (Bismuth, 1995). Minor poisoning produces an obstructive syndrome, atrophic pneumopathy, emphysema, and interstitial fibrosis with disturbances in pulmonary mechanics and diffusion (Gervais, 1975). Patients who survive may have progressive improvement in pulmonary function (Bismuth & Hall, 1995; Lee et al, 1995).
- Early fatalities from paraquat poisoning usually occur after ingestion of larger doses (more than 40 to 50 mg paraquat ion per kg) and may be due to shock, pulmonary damage, or renal failure (EPA, 1985; Bismuth, 1995); such patients may die in 24 hours or less (EPA, 1985; Bismuth, 1995).
- The selective action of paraquat on the lung is thought to be due to formation of an oxygen-attracting radical, resulting in the formation of unstable hydroperoxides which react with alveolar tissue (Jaeger at al, 1995). In a fatal acute human poisoning, arterioles, venules, and terminal bronchioles were unaffected by the poisoning and served as centers for reparative processes, leading to pulmonary fibrosis (Nienhaus & Ehrenfeld, 1971).
CHRONIC CLINICAL EFFECTS
- At the time of this review, no studies were found on chronic exposure to paraquat methosulfate itself were found in humans or experimental animals.
- Occupational exposure to PARAQUAT has resulted in skin irritation or burns, eczematous dermatitis, and nail damage (Hall & Becker, 1995; Garnier, 1995). Two cases of inhalation exposure involved symptoms of cough with sputum production, chest tightness, fever, malaise, and wheezing (Hall & Becker, 1995). Inhalation exposure to paraquat during spraying operations generally does NOT result in systemic toxicity, as the droplets generated by most spraying equipment are too large to be respirable (Hall & Becker, 1995).
- Detailed health surveys in plantation workers occupationally exposed to paraquat for up to 20 years have failed to demonstrate chronic effects. There were no clinically significant differences between paraquat users and non-exposed controls on medical examinations, blood tests, lung function measurements, and chest x-rays (Howard et al, 1981; Senanayake et al, 1993).
