Medical uses include inhalation to produce selective pulmonary vasodilation and improve oxygenation in patients with various forms of pulmonary hypertension. Occupationally, nitric oxide is used in the manufacture of nitric acid, in the bleaching of rayon, and as a raw material for nitrosyl halide preparation (Sittig, 1991).
-CLINICAL EFFECTS
GENERAL CLINICAL EFFECTS
- USES: Medical uses include inhalation to produce selective pulmonary vasodilation and improve oxygenation in patients with severe pulmonary hypertension. Occupationally, nitric oxide is used in the manufacture of nitric acid, in the bleaching of rayon, and as a raw material for nitrosyl halide preparation.
- TOXICOLOGY: Nitric oxide gas is an asphyxiant which may cause pulmonary toxicity. In the presence of oxygen, it is oxidized to nitrogen dioxide and higher oxides of nitrogen which can produce severe pulmonary toxicity. Nitric oxide is a skin, eye, mucous membrane, and irritant; due to the formation of nitric and nitrous acid, which can permanently alter proteins. Because this reaction is relatively slow, permanent tissue injury may possibly be prevented by IMMEDIATE decontamination. Pulmonary edema may develop after a delay of 4 to 72 hours. Following inhalation, nitric oxide can combine with hemoglobin to form nitrosylhemoglobin; this is rapidly oxidized to methemoglobin; resulting in reduction of oxygen transport by red blood cells. Methemoglobin of more than 1.5 g/dL can cause central and peripheral cyanosis. Nitric oxide is a potent endogenous vasodilator and inhibitor of platelet aggregation.
- EPIDEMIOLOGY: Nitric oxide can be toxic by inhalation and absorption through the skin or eyes. Oral exposure is quite rare, since this material is a gas under ambient conditions.
MILD TO MODERATE TOXICITY: Nitric oxide exposure causes skin, eye, mucosal, and respiratory irritation. Initial signs/symptoms may include coughing and burning in the throat and chest, nausea, and shortness of breath. SEVERE TOXICITY: In severe cases, hypotension, pulmonary injury, and methemoglobinemia may be present; latent symptoms may include nervousness, rapid and shallow breathing, cyanosis, mental confusion, and finally loss of consciousness. Hyperpnea and dyspnea may be seen after some delay. Rapid and shallow respirations, coughing and burning in the throat and chest, and physical signs of pulmonary edema may develop; pulmonary edema may develop after a delay of 4 to 72 hours. EFFECTS IN PREGNANCY: Methemoglobin inducers are considered especially hazardous to the fetus. This is because fetal hemoglobin is more readily oxidized than an adult, because it is converted back to hemoglobin more slowly than an adult, and also because the need for oxygen in the developing fetus is more critical than in the adult.
ADVERSE EFFECTS: Administration of nitric oxide can cause severe systemic hypotension in newborns with persistent pulmonary hypertension associated with severe left ventricular dysfunction. Abrupt withdrawal of therapeutic nitric oxide may result in hypoxemia, oxygen desaturation, and increased pulmonary hypertension.

FIRST AID AND PREHOSPITAL TREATMENT
INHALATION: Move patient to fresh air. Monitor for respiratory distress. DERMAL: Remove contaminated clothes and wash exposed area extremely thoroughly with soap and water. Treat dermal irritation or burns with standard topical therapy. Patients developing dermal hypersensitivity reactions may require treatment with systemic or topical corticosteroids or antihistamines. OCULAR: Remove contact lenses and irrigate exposed eyes with copious amounts of room temperature water for at least 15 minutes. If irritation, pain, swelling, lacrimation or photophobia persist, the patient should be seen in a healthcare facility. INGESTION: Oral exposure is UNLIKELY to occur since this agent is a gas under ambient conditions. Normally exposure causes irritation to oral and respiratory mucosa, emesis is contraindicated. There is no known data to suggest the efficacy of nitric oxide binding to activated charcoal. PERSONNEL PROTECTION: Rescuers must be protected from inhalation by self-contained breathing and from contact by plastic gloves.

Move victim to fresh air. Call 911 or emergency medical service. Give artificial respiration if victim is not breathing. Do not use mouth-to-mouth method if victim ingested or inhaled the substance; give artificial respiration with the aid of a pocket mask equipped with a one-way valve or other proper respiratory medical device. Administer oxygen if breathing is difficult. Clothing frozen to the skin should be thawed before being removed. Remove and isolate contaminated clothing and shoes. In case of contact with substance, immediately flush skin or eyes with running water for at least 20 minutes. Keep victim warm and quiet. Keep victim under observation. Effects of contact or inhalation may be delayed. Ensure that medical personnel are aware of the material(s) involved and take precautions to protect themselves.

-RANGE OF TOXICITY
MINIMUM LETHAL EXPOSURE
Immediately Dangerous to Life or Health: 100 ppm (NIOSH , 1996). Fatal: 200 to 700 ppm (OHM/TADS , 1996).
MAXIMUM TOLERATED EXPOSURE
One hundred to 150 ppm is dangerous for short exposures; 60 to 150 ppm is immediately irritating (OHM/TADS , 1996). Nitric oxide concentrations of 80 ppm or higher may be toxic to infants when administered with 90% oxygen; this has been noted in 8 infants in a controlled study (Miller et al, 1994). Inhaling nitric oxide at concentrations up to 128 ppm did not cause clinically significant methemoglobinemia in normal adults (Young et al, 1994). CASE SERIES: In an assessment of 28 bedside nurses and 18 respiratory therapists attending to newborn and pediatric patients receiving NO, it was determined that the highest measured air concentration of NO in the personal breathing zones were 6.7 ppm in a one minute period. These levels are well below ACGIH limit values and did not result in adverse health effects to the workers (Phillips et al, 1999).

Wear positive pressure self-contained breathing apparatus (SCBA). Wear chemical protective clothing that is specifically recommended by the manufacturer. It may provide little or no thermal protection. Structural firefighters' protective clothing provides limited protection in fire situations ONLY; it is not effective in spill situations where direct contact with the substance is possible.

POTENTIAL FIRE OR EXPLOSION HAZARDS - EMERGENCY RESPONSE GUIDEBOOK, GUIDE 124 (ERG, 2004) Substance does not burn but will support combustion. Vapors from liquefied gas are initially heavier than air and spread along ground. These are strong oxidizers and will react vigorously or explosively with many materials including fuels. May ignite combustibles (wood, paper, oil, clothing, etc.). Some will react violently with air, moist air and/or water. Cylinders exposed to fire may vent and release toxic and/or corrosive gas through pressure relief devices. Containers may explode when heated. Ruptured cylinders may rocket.
Burns only when heated with hydrogen (Budavari, 1996). Cylinders may explode in the heat of a fire (Budavari, 1996).

EXPLOSION HAZARD
- Nitric oxide is capable of detonation in all three physical phases (NFPA, 1994; (Lewis, 1993).
- Carbon disulfide and nitric oxide react explosively with light emission (NFPA, 1994).
- Mixtures of ozone and nitric oxide explode even with small quantities of ozone (NFPA, 1994; (Lewis, 1993).

REACTIVITY HAZARD
- At room temperature and above, nitric oxide reacts with oxygen to emit highly toxic fumes of nitrogen oxides (especially nitrogen dioxide) (OHM/TADS , 1996).

Data presented from the Emergency Response Guidebook Table of Initial Isolation and Protective Action Distances are for use when a spill has occurred and there is no fire. If there is a fire, or if a fire is involved, evacuation information presented under FIRE - PUBLIC SAFETY EVACUATION DISTANCES should be used. Generally, a small spill is one that involves a single, small package such as a drum containing up to approximately 200 liters, a small cylinder, or a small leak from a large package. A large spill is one that involves a spill from a large package, or multiple spills from many small packages. Suggested distances to protect from vapors of toxic-by-inhalation and/or water-reactive materials during the first 30 minutes following the spill.

SPILL OR LEAK PRECAUTIONS - EMERGENCY RESPONSE GUIDEBOOK, GUIDE 124 (ERG, 2004) Fully encapsulating, vapor protective clothing should be worn for spills and leaks with no fire. Do not touch or walk through spilled material. Keep combustibles (wood, paper, oil, etc.) away from spilled material. Stop leak if you can do it without risk. Use water spray to reduce vapors or divert vapor cloud drift. Avoid allowing water runoff to contact spilled material. Do not direct water at spill or source of leak. If possible, turn leaking containers so that gas escapes rather than liquid. Prevent entry into waterways, sewers, basements or confined areas. Isolate area until gas has dispersed. Ventilate the area.
RECOMMENDED PROTECTIVE CLOTHING - EMERGENCY RESPONSE GUIDEBOOK, GUIDE 124 (ERG, 2004) Wear positive pressure self-contained breathing apparatus (SCBA). Wear chemical protective clothing that is specifically recommended by the manufacturer. It may provide little or no thermal protection. Structural firefighters' protective clothing provides limited protection in fire situations ONLY; it is not effective in spill situations where direct contact with the substance is possible.
Isolate the area of the leak. Ventilate the area or release to disperse gas. If source is a cylinder and leak cannot be stopped in place, remove cylinder to safe place in open area and repair leak or allow cylinder to empty (HSDB , 1996). Hazardous Waste Disposal Method: Standing behind a body shield, sprinkle onto a thick layer of 50/50 soda ash-slaked lime. Mix while continuously spraying water with an atomizer. Sift cautiously into large volume of water. Neutralize and route to sewage plant (OHM/TADS , 1996).
