USES/FORMS/SOURCES
Ferrous chloride is used in metallurgy; as a reducing agent; in pharmaceutical preparations; as mordant in dyeing; in the manufacture of ferric chloride; and, in sewage treatment (Budavari, 1989; Sax & Lewis, 1987).
ANHYDROUS FORM White rhombohedral crystals. It may sometimes have a green tint (Budavari, 1989). It is very hygroscopic (Budavari, 1989). Can be sublimed in a stream of HCl at about 700 degrees C (Budavari, 1989). Forms ferric chloride (FeCl3) and Fe2O3 on heating in air (Budavari, 1989).
DIHYDRATE FORM White monoclinic crystals with pale green tint (Budavari, 1989). Loses one H2O at 120 degrees C (also reported to lose one H2O at 150 to 160 degrees C) (Budavari, 1989).
TETRAHYDRATE FORM Pale green to blue-green, monoclinic crystals or crystalline powder (Budavari, 1989). Loses two H2O at about 105 to 115 degrees C (Budavari, 1989). Aqueous solutions are readily oxidized (Budavari, 1989).
-CLINICAL EFFECTS
GENERAL CLINICAL EFFECTS
- Ferrous chloride is corrosive. It is probably an irritant to the eyes, skin, and mucous membranes. Mutation data have been reported for ferrous chloride.
- The following information is for IRON SALTS.
Inhalation of ferric salts as dusts and mists is irritating to the respiratory tract. Ferric salts are regarded as skin irritants. Ingestion of soluble salts cause marked gastrointestinal irritation. Eye contact with acidic iron salts such as the sulfate or chloride has caused transient irritation and inflammation, owing principally to their acidity. On prolonged contact with the conjunctiva, they have been known to cause a local brown discoloration. Signs and symptoms of severe poisoning with large amounts of ferrous salts consist of abdominal pain, diarrhea, or vomiting brown or bloody stomach contents, pallor or cyanosis, lassitude, drowsiness, hyperventilation due to acidosis, and cardiovascular collapse. If death does not occur within 6 hours, there may be a transient period of apparent recovery, followed by death in 12 to 24 hours. The corrosive injury to the stomach may result in subsequent pyloric stenosis or gastric scarring. Hemorrhagic gastroenteritis and hepatic damage are prominent findings at autopsy. Although iron salts differ considerably in astringency, lethal doses appear to be closely related to the total iron content.

MAJOR - Stupor, shock, acidosis, hematemesis, bloody diarrhea or coma MINOR - Vomiting, diarrhea, mild lethargy, and hyperglycemia
- CLINICAL COURSE (May Not Occur In All Cases)
PHASE I (0.5 to 2 h): Vomiting, abdominal pain, diarrhea, hematemesis, lethargy, shock, hematochezia, acidosis and coagulopathy PHASE II (after phase I): Apparent recovery may contribute to a false sense of security. Observe intensively. PHASE III (2 to 12 hours after phase I): Profound shock, severe acidosis, cyanosis and fever PHASE IV (2 to 4 days): Possible hepatotoxicity PHASE V (days to weeks): GI scarring; strictures.

TOXIC; inhalation, ingestion or skin contact with material may cause severe injury or death. Contact with molten substance may cause severe burns to skin and eyes. Avoid any skin contact. Effects of contact or inhalation may be delayed. Fire may produce irritating, corrosive and/or toxic gases. Runoff from fire control or dilution water may be corrosive and/or toxic and cause pollution.

SUMMARY

Move victim to fresh air. Call 911 or emergency medical service. Give artificial respiration if victim is not breathing. Do not use mouth-to-mouth method if victim ingested or inhaled the substance; give artificial respiration with the aid of a pocket mask equipped with a one-way valve or other proper respiratory medical device. Administer oxygen if breathing is difficult. Remove and isolate contaminated clothing and shoes. In case of contact with substance, immediately flush skin or eyes with running water for at least 20 minutes. For minor skin contact, avoid spreading material on unaffected skin. Keep victim warm and quiet. Effects of exposure (inhalation, ingestion or skin contact) to substance may be delayed. Ensure that medical personnel are aware of the material(s) involved and take precautions to protect themselves.
INHALATION EXPOSURE INHALATION: Move patient to fresh air. Monitor for respiratory distress. If cough or difficulty breathing develops, evaluate for respiratory tract irritation, bronchitis, or pneumonitis. Administer oxygen and assist ventilation as required. Treat bronchospasm with an inhaled beta2-adrenergic agonist. Consider systemic corticosteroids in patients with significant bronchospasm.
DERMAL EXPOSURE EYE EXPOSURE DECONTAMINATION: Remove contact lenses and irrigate exposed eyes with copious amounts of room temperature 0.9% saline or water for at least 15 minutes. If irritation, pain, swelling, lacrimation, or photophobia persist after 15 minutes of irrigation, the patient should be seen in a healthcare facility.
ORAL EXPOSURE ACIDS The following information is for Acids in general. Do not induce vomiting, do not give bicarbonate to neutralize. Activated charcoal is of no value. Passing a nasogastric or orogastric tube into the stomach is controversial at this time. Irrigate all contaminated areas with copious amounts of water. ORAL EXPOSURE DILUTION - Immediately dilute with 4 to 8 ounces (120 to 240 mL) of milk or water in adults and 2 to 4 ounces in children. STEROIDS - Steroid use is controversial. There is no good data in the literature to assess the efficacy of steroids for oral burns. In severe cases of gastrointestinal necrosis or perforation, surgical consultation should be obtained.
EYE EXPOSURE DECONTAMINATION: Remove contact lenses and irrigate exposed eyes with copious amounts of room temperature 0.9% saline or water for at least 15 minutes. If irritation, pain, swelling, lacrimation, or photophobia persist after 15 minutes of irrigation, the patient should be seen in a healthcare facility.
DERMAL EXPOSURE
IRON The following information is for Iron. MAJOR CLINICAL FINDINGS PRESENT Monitor electrolytes carefully, correct shock. Blood products may be necessary. HYPOTENSION - Administer IV fluids and place patient supine. If unresponsive to these measures, administer dopamine (2 to 5 mcg/kg/min) (first choice) or norepinephrine (0.1 to 0.2 mcg/kg/min) and titrate as needed to desired response. Institute deferoxamine therapy as outlined. Consider exchange transfusion in severely symptomatic patients with a serum iron exceeding 1000 mcg/dL (179.1 mcmol/L).
MINOR OR NO CLINICAL FINDINGS PRESENT IRON INGESTED Asymptomatic patient - (i) Obtain abdominal and chest x-ray. (ii) Negative x-ray - Observe for 6 hours; if asymptomatic then toxicity is unlikely. (iii) Positive x-ray - Evacuate gastric contents and repeat x-ray. Obtain CBC, blood sugar, and serum iron (SI). Institute deferoxamine therapy if SI peak SI is more than 350 mcg/dL (62.6 mcmol/L) or the patient becomes symptomatic. Symptomatic patient: (i) Obtain abdominal and chest x-ray; and evacuate stomach. (ii) Determine serum iron (SI), CBC, and blood sugar. SI may be most useful 3 to 4 hours post-ingestion for liquids/tablets. An additional level 3 to 4 hours later may be helpful with sustained-release or enteric-coated products. (iii) Institute deferoxamine therapy if peak the patient develops more than minor GI irritation or SI exceeds 350 mcg/dL (62.6 mcmol/L).
