GENERAL CLINICAL EFFECTS
- Ethylene fluorohydrin is a liquid fluoro alcohol compound which is miscible in water. Little specific data were available specifically about the toxicity of ethylene fluorohydrin; its toxicity is expected to be similar to that of FLUOROACETATE, as it is oxidized to fluoroacetate by tissue alcohol dehydrogenase.
Ethylene fluorohydrin may be absorbed following ingestion, inhalation, or dermal contact. It is used as a rodenticide, although it is not registered for use as a pesticide in the US. The following review discusses the toxicity and treatment of poisoning with FLUOROACETATE.
- Clinical effects are usually seen within 1/2 hour of exposure. Symptoms of nausea, vomiting, excessive salivation, abdominal pain, numbness, a tingling sensation, and apprehension are seen initially, and may last for up to 6 hours. Muscular twitching, blurred vision, and hypotension may develop.
Severe effects such as coma, convulsions, and cardiac arrhythmias may be delayed in onset as long as 20 hours. One death due to subacute fluoroacetate poisoning has been reported. The cardiac effects noted include tachycardia, ventricular fibrillation, and sudden onset of asystole. Death may occur from respiratory depression and hypoxia during convulsions or cardiac arrest. Neurologic sequelae and acute renal failure have been described after acute poisoning. Metabolic acidosis, hyperglycemia, hyperuricemia, elevated levels of hepatic transaminases, and elevated serum creatinine may occur in fluoroacetate poisoning.
- At least one case of severe poisoning with numbness and tingling of the face, excessive salivation, blurred vision, peripheral paresthesias, convulsions, and coma has occurred from inhalation and dermal contact with fluoroacetate. In general, fluoroacetate is absorbed following ingestion and inhalation, but not through intact skin.
- Fluoroacetate mimics acetic acid and reacts with coenzyme A and oxaloacetic acid, forming fluorocitric acid which enters and blocks the Kreb's cycle, allowing accumulation of citric acid.

TOXIC; inhalation, ingestion or skin contact with material may cause severe injury or death. Contact with molten substance may cause severe burns to skin and eyes. Avoid any skin contact. Effects of contact or inhalation may be delayed. Fire may produce irritating, corrosive and/or toxic gases. Runoff from fire control or dilution water may be corrosive and/or toxic and cause pollution.
ACUTE CLINICAL EFFECTS
- Based on its acute oral LD50 of 5 mg/kg in rats (RTECS , 1993), ethylene fluorohydrin is a HIGHLY TOXIC substance. Ethylene fluorohydrin may be absorbed following ingestion, inhalation, or dermal contact (EPA, 1985; Sax & Lewis, 1989). A milligram of pure fluoroacetate is probably enough to cause severe toxicity, and less may be toxic. A dose of 2 to 10 mg/kg (140 to 700 mg for a 70-kg adult) may be fatal (Egekeze & Oehme, 1979; Baselt & Cravey, 1989). One death due to subacute fluoroacetate poisoning has been reported (Peters et al, 1981).
- Clinical effects of fluoroacetate poisoning are usually seen within 1/2 hour of exposure (HSDB , 1993; Grant, 1986). Some or all of the clinical effects of ethylene fluorohydrin may be delayed due to the time required for metabolism to fluoroacetate. Fluoroacetate produces initial symptoms of nausea, vomiting, excessive salivation, abdominal pain, numbness, a tingling sensation, and apprehension, which may last for up to 6 hours (HSDB , 1993; Grant, 1986; Ellenhorn & Barceloux, 1988). Muscular twitching, blurred vision, and hypotension may develop (HSDB , 1993; Grant, 1986).

- Acute fluoroacetate poisoning affects the heart. The cardiac effects noted include tachycardia, ventricular fibrillation, and sudden onset of asystole (HSDB , 1993; Ellenhorn & Barceloux, 1988). Other effects described from acute poisoning by fluoroacetate are delayed neurotoxicity, acute kidney failure, metabolic acidosis, hyperglycemia, hyperuricemia, elevated levels of hepatic transaminases, and elevated serum creatinine (McTaggart, 1970; Trabes et al, 1983; Baselt & Cravey, 1989; Ellenhorn & Barceloux, 1988; Chung, 1984).
- Fluoroacetate mimics acetic acid and reacts with coenzyme A and oxaloacetic acid, forming fluorocitric acid which enters and blocks the Krebs cycle, with accumulation of CITRIC ACID as the result (HSDB , 1993; Baselt & Cravey, 1989).
CHRONIC CLINICAL EFFECTS
- At the time of this review, no studies on chronic exposure to ethylene fluorohydrin were found in humans or experimental animals.

Move victim to fresh air. Call 911 or emergency medical service. Give artificial respiration if victim is not breathing. Do not use mouth-to-mouth method if victim ingested or inhaled the substance; give artificial respiration with the aid of a pocket mask equipped with a one-way valve or other proper respiratory medical device. Administer oxygen if breathing is difficult. Remove and isolate contaminated clothing and shoes. In case of contact with substance, immediately flush skin or eyes with running water for at least 20 minutes. For minor skin contact, avoid spreading material on unaffected skin. Keep victim warm and quiet. Effects of exposure (inhalation, ingestion or skin contact) to substance may be delayed. Ensure that medical personnel are aware of the material(s) involved and take precautions to protect themselves.

The following information is for a related compound, FLUOROACETATE-1080 - A milligram of pure compound is probably enough to cause severe toxicity and less may be toxic. Animal extrapolation to man indicates a dose of 2 to 10 mg/kg may produce death in humans (Egekeze & Oehme, 1979). A death was reported after ingestion of 465 mg or more in an adult (Harrisson et al, 1952).
