AHA is a urease inhibitor used clinically in the prevention of renal calculi (kidney stones), which can arise from high ammonium levels caused by bacteria in urinary tract infections (Bailie et al, 1986). It also has been used as an experimental cancer chemotherapy agent in rodents (Chaube & Murphy, 1968).

ACUTE CLINICAL EFFECTS
AHA is rapidly absorbed from the gastrointestinal tract and peak blood levels occur in 15 to 60 minutes (Feldman et al, 1978). The plasma half-life is 5 to 10 hours in persons with normal kidney function (Feldman et al, 1978). No information was available about absorption by other routes of exposure. AHA can chelate IRON in the gastrointestinal tract (Product Info, 1991).
With an LD50 of greater than 2 gm/kg by the intraperitoneal exposure route in rats, AHA is not acutely toxic in experimental animals (RTECS , 1995). AHA is metabolized in the liver to ACETAMIDE in rats and mice (Feldman et al, 1978).
CHRONIC CLINICAL EFFECTS
The most exposure to acetohydroxamic acid is during chronic therapeutic treatment for struvite nephrolithiasis (recurrent kidney stones in persons infected with organisms which split urea to form ammonia). The normal therapeutic dose is 10 to 15 mg/kg/day, as three or four 250 mg tablets (Product Info, 1993). Adverse effects reported in such patients include headache, nausea, loss of appetite, malaise, tremor, depression, anxiety, confusion, decrease in circulating red blood cells/hemoglobin/hematocrit, hemolytic anemia, and thrombosis (Rodman et al, 1987; (Griffith et al, 1979; Griffith et al, 1978; Burr & Nuseibeh, 1983) Product Info, 1991). Acetohydroxamic acid-induced thrombosis has been characterized by increased levels of plasma fibrinopeptide A (a product of the action of thrombin), which becomes elevated within 24 hours after treatment initiation (Rodman et al, 1987). In one study, documented thrombosis occurred in 17% of patients receiving chronic AHA therapy (Rodman et al, 1987). Platelet counts were also decreased within 5 days, possibly due to formation of microaggregates or thrombi (Rodman et al, 1987). Other (less common) effects reported with chronic treatment include superficial phlebitis of the lower extremities (Griffith et al, 1979), deep vein thrombosis, pulmonary embolism (Williams et al, 1984), palpitations (Product Info, 1991), skin rash (especially after alcohol ingestion) (Product Info, 1991), and alopecia (hair loss) (Product Info, 1991).
