-CLINICAL EFFECTS
GENERAL CLINICAL EFFECTS
- Coumatetralyl is a derivative of 4-hydroxycoumarin. In massive overdose, these agents have produced rapid and persistent hypoprothrombinemia and associated bleeding diathesis. Once coagulation defects occur, they may persist for 6 weeks to many months. Because it has an elimination half-life of several days, daily exposures would produce cumulative toxicity. No chronic data were found for coumatetralyl in humans, however. This review is based on the properties of coumarin and other long-acting anticoagulants.

Highly toxic, may be fatal if inhaled, swallowed or absorbed through skin. Avoid any skin contact. Effects of contact or inhalation may be delayed. Fire may produce irritating, corrosive and/or toxic gases. Runoff from fire control or dilution water may be corrosive and/or toxic and cause pollution.
ACUTE CLINICAL EFFECTS
- Coumatetralyl may be poisonous by inhalation, dermal, and oral exposure routes (EPA, 1985).
- Hemorrhage is the most common toxic sign and may be manifested by ecchymoses (bruising), epistaxis (nosebleed), gum bleeding, hemoptysis (blood-stained sputum), hematuria (blood in the urine), gastrointestinal bleeding, prolonged prothrombin time (PT), and intracranial hemorrhage. Cardiopulmonary injury has been observed in experimental animals, but has not been reported in coumatetralyl-exposed humans.
- Once coagulation defects develop, they may persist for 6 weeks to many months.
CHRONIC CLINICAL EFFECTS
- Because repeated exposure may progressively inhibit coagulation, coumatetralyl has CUMULATIVE TOXICITY. Coumarin, a related compound, may be a mild allergen upon contact with the skin or mucous membranes (HSDB , 1991).
- Lameness and paralysis were observed in experimental animals poisoned by coumatetralyl (Dobson, 1973).

Move victim to fresh air. Call 911 or emergency medical service. Give artificial respiration if victim is not breathing. Do not use mouth-to-mouth method if victim ingested or inhaled the substance;give artificial respiration with the aid of a pocket mask equipped with a one-way valve or other proper respiratory medical device. Administer oxygen if breathing is difficult. Remove and isolate contaminated clothing and shoes. In case of contact with substance, immediately flush skin or eyes with running water for at least 20 minutes. For minor skin contact, avoid spreading material on unaffected skin. Keep victim warm and quiet. Effects of exposure (inhalation, ingestion or skin contact) to substance may be delayed. Ensure that medical personnel are aware of the material(s) involved and take precautions to protect themselves.
GENERAL The following treatment information is for Anticoagulants - Long Acting. Administer activated charcoal after recent significant ingestion. Vitamin K and transfusions with either whole blood or fresh frozen plasma may be required. Vitamin K therapy may need to be continued for weeks or months in severe cases. Careful and prolonged monitoring of coagulation status and followup is required. Although human cases have not been reported, coumatetralyl ingestion most likely would produce long lasting anticoagulation similar to that of other 4-hydroxy coumarin derivatives. In the absence of full data, it is prudent to evaluate patients with coumatetralyl exposure as though it can produce long lasting anticoagulant effects and to treat actual findings as indicated. DERMAL EXPOSURE EYE EXPOSURE DECONTAMINATION: Remove contact lenses and irrigate exposed eyes with copious amounts of room temperature 0.9% saline or water for at least 15 minutes. If irritation, pain, swelling, lacrimation, or photophobia persist after 15 minutes of irrigation, the patient should be seen in a healthcare facility.
ORAL EXPOSURE ACTIVATED CHARCOAL: Administer charcoal as a slurry (240 mL water/30 g charcoal). Usual dose: 25 to 100 g in adults/adolescents, 25 to 50 g in children (1 to 12 years), and 1 g/kg in infants less than 1 year old. VITAMIN K1 (PHYTONADIONE) - Is a specific antidote and should be administered to any patient with a prolonged INR/PT. Menadione (vitamin K3) should NOT be used. Oral therapy may be indicated in small ingestions or when the amount is uncertain. Administer 15 to 25 mg in adult and 5 to 10 mg in children. Daily maintenance doses of 100 to 125 mg/day may be required for 1.5 to 8 months. IV injection is preferable in severe cases where rapid correction is required. Dose - Adult - 10 mg IV diluted in saline or glucose at a rate not exceeding 5% of the total dose/min. IM injection may be indicated in mild ingestions where the risk of hematoma is low, Dose - Adult - 5 to 10 mg. Child - 1 to 5 mg. There is no specific therapeutic maneuver other than restoration of INR/PT level to normal if toxicity occurs. Fresh blood or plasma is the most rapid, effective method of stopping hemorrhage. PATIENTS PRESENTLY ON ANTICOAGULANT THERAPY - Get PT or INR immediately. If patient is anticoagulated for prosthetic valve or other procedure requiring absolute anticoagulation, do not give vitamin K unless anticoagulation is excessive. Give a small IV dose (1 to 5 mg and titrate to return PT to therapeutic). Substitution of heparin as an anticoagulant may be necessary until PT is therapeutic.
