		Planned surgery is high risk type (1 point)
		Patient has coronary artery disease (1 point)
		Patient has history of CHF (1 point)
		Patient has history of cerebrovascular disease (1 point)
		Paient has diabetes and uses insulin (1 point)
		Chronic renal insufficiency is present with Cr > 2 mg% (1 point)
